Exhibit C: Annual Financial Report Instructions and Form
Exhibit C Overview
A signed paper copy of Exhibit C: Annual Financial Report for Contract or Extension by Amendment is due from each grant recipient's fiscal or contracts and grants office every September 30.
The form is called Exhibit C because it is the third "exhibit" attached to the contract each site receives when it first becomes an NWP site. The contract is between NWP and the site's host institution. Each host institution is contractually responsible for filing the Exhibit C with NWP every year. Please note: New-site host institutions are not required to turn in an Exhibit C report until a full program year has been completed.
The data for this report come from the records kept in the host institution's fiscal offices. In the case of an audit, these are the data that will be audited.

Submitting the Exhibit C
An Excel file of the Exhibit C is available above for downloading. The completed Exhibit C must be printed and signed by a university financial officer and the project director.
The signed report should be submitted to NWP by September 30 using either option below:
- Email a scanned PDF to: bhasselbach@nwp.org
- Mail the signed original to:
